  • Fill out required forms prior to consultation appointment (Medical History, HIPPA, Consent to Treat).

Step #2 - Discuss format options with NP and select your preferred Weight Loss Plan.
  • Plans vary based on your health history and weight loss goals.
  • Choose your preferred format: Telehealth only or In-person Course + Telehealth.
  • Medication is included with approved plans.
  • All plans include a comprehensive approach: One-On-One Coaching, Healthy Eating – Exercise – Lifestyle Changes through Behavior Modifications.

Step #3 - Complete all lab tests ordered by the NP.
  • Lab results from your healthcare provider within the last 6 months will be accepted.

Step #4 - Create your Weight Loss Plan.
  • Set realistic goals to improve your current eating habits.
  • Set reasonable exercise and physical activity goals. Plan ways to fit regular physical activity into your daily routine.
  • Track Your Progress: Your Practice Better Client Portal provides all the tools necessary to help you succeed.

Step #5 - Remember, the key to successful and sustainable weight loss is adopting a healthy lifestyle through small changes one day at a time.
